“Particulars of Persons who will be Deployed to Conduct Exit Poll” is the table in “Attachment I” of “Application Form for Conduct of Exit Poll” (REO/EP/1/2025/ECSS(By-E)). Before uploading, please read carefully “Attachment II - Guidance Notes on Completion of Application Form” of the “Application Form for Conduct of Exit Poll”.

Personal Information Collection Statement

(a) Purpose of Collection

The personal data and other information supplied in connection with this Application Form will be used by the EAC and the REO for election-related purposes. A list of persons or organisations allowed to conduct exit polls and their contact telephone numbers, together with the names of their interviewers will be released prior to the polling day for reference by the public and candidates. Such a list will also be displayed at each of the relevant polling stations respectively. All the identity document numbers and the declarants’ addresses will be covered up before the lists are displayed.

(b) Transfer of Information

The personal data and other related information supplied in connection with this Application Form may be provided to the relevant law enforcement agencies, other authorised departments or persons, including (but not limited to) the law courts for election-related purposes.

(c) Access to Personal Data

A person has the right to request access to and correction of the personal data he / she supplied in accordance with the provisions of the Personal Data (Privacy) Ordinance (Cap. 486).

(d) Personal Data Protection Principles

Please note that applicants must observe the Data Protection Principles in Schedule 1 to the Personal Data (Privacy) Ordinance (Cap. 486) in collecting personal data of a third person in completing this Application Form, in particular Principle 4 concerning the requirement to take all practicable steps to ensure that the personal data are protected against unauthorised or accidental access, processing and use.

(e) Enquiry
